CHO Training for Small Group Enrollment
There are two methods for enrolling small groups. For very small groups, you may enter the employee information one at a time, or you may set up a new group store within the system. There is no cap on how many employees you can enroll in a group.

Enrollment: Use your “Benefits Store” lower left to enroll customer

Select New Member or Current Member (to make additions to active clients), then Collective Wellness Association, Inc.
▶ Step 4
Enter the employee demographics, submit and scroll down to view products offered through Choice Health.
▶ Step 5
Select More Info to view the product and add to the cart.
▶ Step 6
Next, you can continue shopping and add more products, send the quote for member self-enroll, or proceed to checkout.

Use the employer’s payment method for each employee. These charges will be itemized on the employer’s bank statement.

All enrollee’s must join the Collective Wellness Association, Inc. CWA provides additional benefits for your members found here.

Within checkout, you are required to read the below *Statement provided to you at checkout to your client.
